Leaving Hospital After Treatment

Many LINk Members, and associated community groups, are aware that the Portsmouth LINk is interested in the topic of Discharge from Care. Queen Alexandra Hospital has provided the Portsmouth LINk with a draft leaflet which aims to provide information on the discharge planning pathway for all patients admitted to Queen Alexandra Hospital. Terry Carter, Vice Chair of the Portsmouth LINk, feels that the following information and provisions need to be included in the Draft Discharge Leaflet: (a) Information about 'aids' to assist a patient with mobility difficulties at home e.g. an additional stair rail, a raised mattress, or a raised toilet seat; (b) Information about the provision of 'meals on wheels' at home; (c) Support with medication to include guidance about repeat prescriptions being delivered by the local Pharmacist; (d) Special support for vulnerable adults e.g. people with learning difficulties or people with some memory loss to enable them to understand the Discharge Leaflet and the implications of its questions (and trained staff to deliver this support); (e) “Sign Posting” to Social Workers, employed at the hospital, who need to be involved in aspects of Discharge.

  3. Thank you Jock. I also think it could be useful for those involved with Discharge to have a Check List to hand. Not all items may apply - but it would be a systematic approach.

  4. In addition to previous comments:
    Page 3 Replace jargon "with a package of care" with "after arranging all the care you will need".

    Page 5 Specify the likely maximum number of days medication you will be given (this enables people to know how soon they will have to contact their own GP for prescriptions)
